What is a semen analysis?
A semen analysis is a laboratory test that examines a semen sample to assess sperm health.
The test looks at several factors, including:
How many sperm are present.
How well they move.
What they look like.
The volume of semen produced.
Other characteristics that may affect fertility.
Although it cannot guarantee fertility, it provides an important picture of male reproductive health.
Why is a semen analysis done?
A semen analysis is usually one of the first investigations recommended when a couple has difficulty conceiving.
This is because male factors contribute to infertility in a significant proportion of couples.
Testing both partners early can help avoid unnecessary delays in reaching a diagnosis.
How is the sample collected?
Most samples are collected by masturbation into a sterile container.
The sample is usually produced:
At home (if it can reach the laboratory within the required time), or
In a private room at the fertility clinic or hospital.
You'll normally be advised to avoid ejaculation for 2–7 days before providing the sample, as this helps produce the most reliable results.

What does the laboratory measure?
A semen analysis looks at several different features.
Semen volume
This measures how much semen is produced.
A very low volume may affect the chances of sperm reaching the egg and can sometimes suggest an underlying problem.
Sperm concentration
This is the number of sperm present in each millilitre of semen.
Generally speaking:
Higher numbers increase the chances of fertilisation.
Lower numbers may reduce fertility.
However, pregnancy can still occur naturally with lower sperm counts.
Total sperm count
This is the total number of sperm in the entire sample.
It takes both concentration and semen volume into account.
Sperm motility
Motility describes how well the sperm move.
Healthy sperm need to swim through the cervix, uterus and fallopian tube to reach the egg.
Poor movement may make this journey more difficult.
Sperm morphology
Morphology refers to the shape of the sperm.
The laboratory looks at:
The head
The midpiece
The tail
It's normal for many sperm to have an unusual shape.
Only a small percentage of perfectly formed sperm is needed for natural conception.
Sperm vitality
If many sperm are not moving, the laboratory may check whether they are alive.
This helps distinguish between sperm that are alive but immobile and sperm that are no longer viable.
White blood cells
Large numbers of white blood cells may suggest infection or inflammation.
Further investigations may be recommended if this is found.
What do the results mean?
Results are interpreted as a whole.
One abnormal result does not necessarily mean infertility.
Many factors—including illness, stress, fever and even the length of abstinence before the test—can temporarily affect sperm quality.
For this reason, an abnormal result is often repeated several weeks later before firm conclusions are drawn.
Can sperm quality change?
Yes.
Unlike women, who are born with all of their eggs, men continuously produce new sperm.
It takes around three months for new sperm to develop.
Because of this, lifestyle changes, treatment of medical conditions and recovery from illness may improve sperm quality over time.
Can a normal semen analysis guarantee fertility?
No.
A normal result is reassuring, but it does not guarantee pregnancy.
There are aspects of sperm function that routine semen analysis cannot assess, such as how well sperm interact with or fertilise an egg.
Similarly, a slightly abnormal result does not mean pregnancy is impossible.
Many couples conceive naturally despite mild abnormalities.
